PHYSICAL THERAPY
- Initial evaluation: 60 minute session
- Follow-up treatments: 30 or 60 minute sessions
No referral needed unless directed by your insurance for reimbursement

YOGA & WELLNESS EXERCISE
- Initial assessment: 60 minute session
- Yoga or wellness exercise sessions: 30 or 60 minutes
- Semi-private yoga: 60 minute sessions
- All-levels group yoga classes & events
Investment
Initial Evaluation/Assessment: $175
60 minute sessions: $150
30 minute sessions: $75
Group yoga classes: $25 each or 5-class package $100 (specialized series package prices vary)
Please Note:
Ascend LLC is an out-of-network provider for Physical Therapy with all commercial insurers.
At this time, Ascend LLC is unable to provide Medicare-eligible clients Physical Therapy services. Medicare-eligible clients can be assessed for and receive Yoga or Wellness Exercise out-of-pocket.
Note, if you are a returning client who has not been to the practice in six months or has a change in medical history, an initial evaluation/assessment will be required to ensure proper care and that an updated plan is developed.
